Section 605(c) of
Pub. L. 104–204 provided that: “The amendments made by this section [enacting section
300gg–51 of this title and amending this section and sections
300gg–61 and
300gg–62 of this title] shall apply with respect to health insurance coverage offered, sold, issued, renewed, in effect, or operated in the individual market on or after January 1, 1998.”
