Distributions. This section shall apply to a distribution from a terminated individual account plan if, in connection with such distribution:
(1) The participant or beneficiary, on whose behalf the distribution will be made, was furnished notice in accordance with paragraph (e) of this section or, in the case of an abandoned plan, ยง 2578.1(d)(2)(vi) of this chapter, and
(2) The participant or beneficiary failed to elect a form of distribution within 30 days of the furnishing of the notice described paragraph (b)(1) of this section.
