Ala. Admin. Code r. 465-X-3-.08 - Replacement Licenses
The Board shall issue a replacement license, upon application, to a licensee who shows by notarized statement that:
(1) the licensee's
current license has been stolen, taken, lost, destroyed, defaced,
or
(2) the licensee's
name has been changed by marriage or court order, if the licensee is
an individual, or by changing the name of the sole proprietorship,
partnership, or corporation, if the license is issued in the name of
such an entity. Where such a name change occurs, the license issued
under the licensee's former name shall expire 60 days after the name
change occurs, unless within that 60-day period the licensee obtains
a replacement license bearing the licensee's correct name. When the
replacement license is issued to accommodate a name change, the
originally issued license must be returned to the Board as a
condition of the issuance of the new license.
