12 AAC 28.040 - Informed Written consent
Before administering deep sedation, general anesthesia, or moderate sedation, and before administering minimal sedation to a patient younger than 13 years of age, a dentist shall
(1) discuss with he patient or patient's
parent; legal guardian or caregiven if the patient is younger than 13 yeras of
age, risks, benefits, and alternatives;
(2) obtain written consent of the patient or
of the parent or legal guardian; and
(3) maintain a copy of the informed written
consent in the patient's permanent record.
