3 AAC 14.150 - Application for registration

(a) To apply for registration under AS 06.60.014, a depository institution shall submit

(1) to the registry

(A) a complete Form MU1;

(B) a complete Form MU2 for each control person and branch manager of the applicant;

(C) a financial statement in compliance with 3 AAC 14.054;

(D) a copy of the surety bond in compliance with 3 AAC 14.053; and

(E) the designation of a registered agent under 3 AAC 14.057; and

(2) to the department

(A) if the answer to any of the disclosure questions on the Form MU1 or Form MU2 is "yes," complete details in writing of each event or proceeding, including as applicable, the name and location of the court, the docket or case number, and the status and summary of the event or proceeding, and a copy of an applicable charge, order, or consent agreement; and

(B) other information, documentation, and payments as allowed under AS 06.60 and required by the department.

(b) A registration under AS 06.60.014 remains in effect until December 31 of the year in which the registration is issued.
