Cal. Code Regs. Tit. 13, § 125.16 - Verification of Installation

(a) In addition to any other application requirements, an applicant for an ignition interlock device restricted driver's license shall submit a form DL 920, and the applicable administrative service fee as defined in Section 127.02 of this Article.

(b) A manufacturer shall account for each form DL 920, issued to the manufacturer. If any unused forms become lost, stolen, or misplaced the manufacturer shall immediately notify local law enforcement and provide written notification to the department.
