Cal. Code Regs. Tit. 13, § 125.18 - Notice of Non-Compliance
(a) An installer shall submit to the
department within three working days a form DL 921 if an ignition interlock
device indicates that the participant has attempted to remove, bypass, or
tamper with the device, if the participant fails three or more times to comply
with any requirement for the maintenance or calibration of the ignition
interlock device , or if the participant fails to comply with any requirements
for the maintenance or calibration of the ignition interlock device within 60
days of the previous service appointment.
