Cal. Code Regs. Tit. 22, § 74215 - Fiscal Reports
(a)
Each licensee shall furnish to the Department annually, on or before March 15,
a fiscal report of its activities during the preceding calendar year, on forms
supplied by the Department. The fiscal report shall include, but not be limited
to:
(1) The number of clients
served.
(2) The total amount of
referral fees collected from clients.
(3) The total amount of referral fees
collected from health facilities.
(4) Any income in addition to referral fees,
and its source.
(5) Names and
addresses of the facilities with which the referral agency has done business
during the preceding year.
(6)
Other information, as required by the Department.
