Cal. Code Regs. Tit. 4, § 12080 - Requirements
(a) An applicant, licensee , or holder of a
Commission work permit may designate a natural person(s) to serve as their
designated agent (s) pursuant to Title 11, Cal. Code Regs., Section
2030, using the Appointment of
Designated Agent , CGCC-CH1-04 (Rev. 07/22), which is attached in Appendix A to
this chapter.
(b) A natural
person(s) must be authorized as the applicant's, licensee 's, or holder of a
Commission work permit 's designated agent before representing the applicant,
licensee , or holder of a Commission work permit before the
Commission .
(c) A designated agent
must provide the following, if applicable:
(1)
The designated agent 's California State Bar number indicating a current license
in good standing; or,
(2) The
designated agent 's California Board of Accountancy number indicating a current
license in good standing.
(d) If the designated agent provided a
license number pursuant to subsection (c), then the designated agent must
provide written notification to the Bureau within 5 business days of any change
in the licensee 's standing or any disciplinary action.
