(a) Each program shall maintain in every
patient's file the following information:
(1)
The date the patient body specimen was collected;
(2) The test or analysis results;
and
(3) The date the program
received the results of the test or analysis.
(b) All test or analysis records for
illicit
drug use shall be from a
laboratory in compliance with Section
10320.
