Conn. Agencies Regs. § 10-306-16a - Individuals determined to have achieved an employment outcome
(a) An individual is determined to have
achieved an employment outcome only if the following requirements are met:
(1) The provision of services under the
individual's IWRP has contributed to the achievement of the employment
outcome.
(2) The employment outcome
is consistent with the individual's strengths, resources, priorities, concerns,
abilities, capabilities, interests, and informed choice.
(3) The employment outcome is in the most
integrated setting possible, consistent with the individual's informed
choice.
(4) The individual has
maintained the employment outcome for a period of at least ninety (90)
days.
(5) At the end of the
appropriate period under subdivision (4) of this section, the individual and
the rehabilitation counselor consider the employment outcome to be satisfactory
and agree that the individual is performing well on the job.
