Conn. Agencies Regs. § 19a-401-17 - Form of request for declaratory ruling

A request for declaratory ruling shall be addressed to the Commission and sent to the Chairperson of the Commission by mail to the Office of the Chief Medical Examiner, 11 Shuttle Rd., Farmington, Connecticut 06032 or delivered in person during normal business hours. The request shall be signed by the person in whose behalf the request is made or by such person's attorney The request shall identify the address of the person making the request and the name and address of such person's attorney, if applicable. The request shall clearly and concisely state the substance and nature of the request. It shall identify the statute, the regulation or order concerning which the request is made and shall identify the particular aspect thereof to which the request is directed. The request for a declaratory ruling shall be accompanied by a statement of any data, facts and arguments that support the position of the person making the request.
