Fla. Admin. Code Ann. R. 64E-5.603 - Notification
A licensee shall notify the department in writing within 30 days when the licensee changes its mailing address or when an authorized user, RSO, authorized nuclear pharmacist, or authorized medical physicist permanently discontinues performance of their duties under the licensee.
