Service of
Notice. A notice is considered properly served by any individual, or
organization authorized to serve a civil process notice in any of the following
ways: (7-1-21)T
a. The notice is personally
delivered to the license holder, manager or person in charge of the food
establishment. (7-1-21)T
b. The
notice is clearly posted at a public entrance to the food establishment and a
copy of the notice is also sent by first-class mail to the license holder.

c. The notice is sent to
the license holder's last known address by registered or certified mail, or by
other public means in which a written acknowledgment of receipt is acquired.

Proof
of Service. Proof of service is determined when the person delivering the
notice signs a certificate stating the notice has been served or posted, or by
admission of the signed receipt by the license holder or person in charge of the
food establishment. (7-1-21)T
