ยง 1515.45. [Effective until 10/9/2021]Application for Illinois Short-Term Conditional License

a) An applicant for an Illinois Short-Term Conditional License shall file an application on the forms prescribed by the Commission, together with the following:

1) Proof of transcript showing completion of ASL Interpretation coursework from an Illinois Interpreter Training Program issued during the Gubernatorial Disaster Proclamations;

2) Signed affidavit that the applicant for the Illinois Short-Term Conditional License will complete one of the accepted certifications as specified in Section 1515.50 within 12 months of their Illinois Short-Term Conditional License being approved; and

3) The required fee set forth in Section 1515.70.

b) An Illinois Short-Term Conditional License shall expire 12 months from the date of issuance. Under no circumstances shall a temporary license be issued for a time longer than 12 months. An Illinois Short-Term Conditional License shall automatically expire upon issuance of the Illinois License.
