Ill. Admin. Code tit. 14, § 178.180 - Assumed Names
a) A limited
liability company may adopt, change, renew or cancel its assumed name by filing
an application with the Department and paying the fee as prescribed in Section
5-10 of the LLCA. The application shall contain the following required
information: the name of the limited liability company, the file number, and
the assumed name to be adopted, changed, renewed or cancelled.
b) The application to adopt, change, or
cancel an assumed name shall be on a separate form from the application to
renew an assumed name.
