312 IAC 29-33-6 - Written notice of intent to plug a well and well plugging plan

Sec. 6.
(a) Except as
provided in sections 9 and 10 of this rule, the owner and operator shall give
written notice to the division of intent to plug an existing well at least
fifteen (15) days before commencing the plugging of the well.
(b) The written notice required by subsection
(a) shall be on a form prescribed by the division.
(c) Unless a well plugging plan has been
previously submitted to the division for approval, the notice required under
subsection (b) must include a plan for plugging the well that:
(1) describes the specific methods that would
be used;
(2) specifies the date on
which plugging operations are scheduled to commence, if known; and
(3) indicates compliance with this rule,
including the location of each cement plug to be placed in the well.
