460 IAC 14-10-3 - Case closure for other reasons

Sec. 3.
The record of services for any applicant or eligible individual may be closed at any time for reasons other than ineligibility or achieving an employment outcome including, but not limited to, a counselor determines that:
(1) the individual is
deceased;
(2) the individual is
unavailable to participate in or complete an assessment of eligibility and
priority for services;
(3) the
individual is no longer present in the state;
(4) the individual cannot be located to
begin, continue, or complete vocational rehabilitation services;
(5) the individual has declined
services;
(6) the individual has
been institutionalized and will be unavailable to participate in vocational
rehabilitation services for a lengthy or indefinite period of time;
(7) the individual has been transferred to
another agency or program for services that are more appropriate for the
individual, and the necessary referral has been made and transfer process
completed;
(8) transportation
required to accept or maintain employment is not feasible or
available;
(9) the individual has
made an informed choice to pursue extended employment, and the required
referral has been made;
(10) the
individual has failed to cooperate so that vocational rehabilitation services
cannot begin, continue, or be completed; or
(11) the individual has failed to achieve the
employment outcome described in his or her IPE, and a determination is made
that further services are not likely to result in achievement of the employment
outcome.
