470 IAC 3.1-8-1 - Evaluation and assessment activities

Sec. 1.
(a) Each
child, birth through two (2) years of age, referred to a SPOE shall have a
timely, comprehensive, multidisciplinary evaluation, including assessment
activities related to the child and the child's family. The multidisciplinary
team may utilize evaluations and assessments performed prior to the child's
participation at this level.
(b)
The evaluation and assessment of the child shall be as follows:
(1) Be conducted by personnel trained to
utilize appropriate methods and procedures.
(2) Be based on informed clinical
opinion.
(3) Include the following:
(A) A review of pertinent records related to
the child's current health status and medical history.
(B) An evaluation of the child's level of
functioning in each of the following developmental areas:
(i) Cognitive development.
(ii) Physical development, including vision
and hearing.
(iii) Communication
development.
(iv) Social or
emotional development.
(v) Adaptive
development.
(C) An
assessment of the unique needs of the child in terms of each of the
developmental areas specified in this subdivision, including identification of
services appropriate to meet the needs of the child.
