68 IAC 15-5-9 - Monthly forms

Sec. 9.

(a) The casino licensee shall prepare and submit the following forms to the commission office in Indianapolis, Indiana not later than fifteen (15) days after the end of a calendar month:

(1) Form RG-3.

(2) Form RG-4.

(3) Form RG-5.

(4) Form RG-6.

(5) Form RG-7.

(6) Form RG-8.

(7) Form RG-9.

(b) The casino licensee shall prepare and submit the Form RG-2 to the commission office in Indianapolis, Indiana not later than five (5) days after the end of a calendar month.

(c) The forms listed in this section shall be prepared in accordance with this rule and the instructions that accompany the form. The original forms shall be submitted to the commission office in Indianapolis, Indiana by the prescribed day of a month. Copies of each form shall be submitted to the department in Indianapolis, Indiana. Copies submitted via facsimile are not acceptable as being timely filed.
