Iowa Admin. Code r. 11-64.5 - Health benefit appeals
A member who disagrees with a group health benefit company's decision on the application of group contract benefits may:
1. File a written appeal with the respective
company as defined in the group contract, or
2. File a written appeal with the
commissioner of insurance at the department of commerce.
