In lieu of the words "designate office" insert "Administrator, State Historical Society of Iowa, Capitol Complex, Des Moines, Iowa 50319, (515)281-8837".
In lieu of the words, "AGENCY NAME", the heading on the petition shall read:
BEFORE THE STATE HISTORICAL SOCIETY OF IOWA
