Iowa Code r. 876-6.4 - Forms for commutation
Form No. 9 (14-0013) shall be used for a full commutation. Form No. 9A (14-0017) shall be used for a partial commutation. See rule 876-3.1 (17A) for description of these forms.
This rule is intended to implement Iowa Code sections 85.45 and 85.47.
