Kan. Admin. Regs. § 14-13-7 - Licenses, loss or destruction of; duplicate license
(a) Whenever any license issued by the
director is lost or destroyed before its expiration, the retailer to whom the
license was issued may submit a written application to the director for a
duplicate license.
(b) The
application required by subsection (a) shall be submitted on forms prescribed
by the director and shall contain the facts and circumstances concerning the
loss or destruction of the license.
(c) The director may issue a duplicate
license upon receipt of information that the license has been lost or
destroyed.
