Kan. Admin. Regs. § 51-3-3 - Disapproving final receipt and release of liability
A final receipt and release of liability shall be disapproved by the director unless it meets the following requirements:
(a) The form shall be filled out completely.
(b) The form shall be accompanied by a physician's report, and
the substance of the report shall conform to the information contained in the
final receipt and release of liability.
(c) The form shall show that compensation has been paid in
conformity with the requirements of the act.
(d) The form shall be filed within 60 days of execution.
(e) The form shall be executed within 60 days of the last
payment of compensation.
(f) The form shall have the notarized signature of the
claimant.
