A. A
business entity, organization, or individual requesting approval to provide
driver rehabilitation and education programs under this chapter shall submit to
the Administration an application for approval.
B. An application shall be submitted for each
rehabilitation and education program to be provided and a separate application
is required for each program being presented in an alternate format.
