Mont. Admin. r. 24.174.901 - PATIENT RECORDS AND DRUG UTILIZATION REVIEW
(1) A patient record system shall be
maintained by all pharmacies for patients for whom prescriptions are dispensed.
The patient record system shall provide for the immediate retrieval of
information necessary for the dispensing pharmacist to identify previously
dispensed drugs at the time a prescription is presented for dispensing. The
pharmacist or pharmacy technician shall make a reasonable effort to obtain,
record, and maintain the following information:
(a) full name of the patient;
(b) address and telephone number of the
patient;
(c) patient's age or date
of birth;
(d) patient's
gender;
(e) known allergies or drug
intolerances;
(f) chronic
conditions;
(g) other prescription
or non-prescription medications; and
(h) pharmacist comments relevant to the
individual's drug therapy, including any other information peculiar to the
specific patient or drug.
(2) A pharmacist shall review the patient
record and complete a drug utilization review as defined in
37-7-101, MCA. Upon recognizing
any potential therapeutic problems, the pharmacist shall take appropriate steps
to avoid or resolve the problem which shall, if necessary, include consultation
with the prescriber.
(3) A patient
record shall be maintained for a period of not less than two years from the
date of the last entry in the profile record. This record may be a hard copy or
a computerized form.
