Mont. Admin. R. 24.29.2610 - CERTIFICATION REQUIREMENTS
(1) The
department shall grant SIF certification to an individual when all of the
following requirements are met:
(a) The
applicant has documented the existence of a permanent physical or mental
impairment that adversely impacts the applicant 's employability, in accordance
with ARM 24.29.2607. The applicant 's
permanent impairment may result from a congenital condition, trauma, or
disease. The permanent impairment does not have to be caused by a work-related
injury or occupational disease;
(b)
The medical evidence of applicant 's permanent impairment is not more than six
months old at the time of application;
(c) The medical evidence of permanent
impairment relates directly to the condition identified on the application
form;
(d) A treating physician has
provided written documentation that the applicant is permanently impaired;
and
(e) A treating physician has
provided written documentation of employment restrictions or limitations due to
the applicant 's permanent impairment that demonstrates the impairment presents
a substantial obstacle to employment or reemployment.
