Mont. Admin. r. 24.174.1403 - APPLICATION FORMS
(1) If any person
is required to be registered and is not so registered and is applying for
registration to manufacture or distribute dangerous drugs in Schedules I
through V, the person shall apply on a form prescribed by the board.
(2) If any person is required to be
registered and is not so registered and is applying for registration to
dispense dangerous drugs in Schedules II through V, the person shall apply on a
form prescribed by the board.
(3)
If any person is required to be registered and is not so registered and is
applying for registration to analyze or conduct research with dangerous drugs
in Schedules I through V, the person shall apply on a form prescribed by the
board.
(4) Any licensee applying
for renewal of registration to manufacture or distribute dangerous drugs in
Schedules I through V, the licensee shall apply on a form prescribed by the
board.
(5) Any licensee applying
for renewal to dispense dangerous drugs in Schedules II through V, the licensee
shall apply on a form prescribed by the board.
