N.H. Admin. Code § Ath 402.03 - Required Documents

Applicants for renewal of their licenses shall include in their application packets:

(a) A detailed report of the relevant circumstances if any answers to questions from the application form specified in Ahp 601.03 are in the affirmative; and
(b) A copy of a certificate issued by Board of Certification for Athletic Trainers (BOC) verifying certification.
