N.H. Admin. Code § Ath 402.06 - Required Documents
Applicants for renewal of their licenses shall include in their application packets:
(a) A
detailed report of the relevant circumstances if any answers to questions set
forth in Ath
402.04(e)(2) and appearing on part
one of the application form is in the affirmative;
(b) On a separate sheet, a detailed report of
the relevant circumstances if any answers to questions set forth in
Ath
402.04(k) and appearing on part 2 of
the application form is in the affirmative; and
(c) A photocopy of the front and back of the
BOC wallet card.
