N.H. Admin. Code § Ch 406.01 - Obligation to Keep Board Records Current

A licensee shall notify the board within 30 days of any changes in the licensee's:

(a) Full name;
(b) Physical address;
(c) Mailing address;
(d) Telephone number;
(e) E-mail address, if the licensee has one; or
(f) National provider identifier.
