N.H. Admin. Code § Cor 409.08 - Individual Job Assignment Suspension Form
The "Individual Job Assignment Suspension Form" shall contain the following:
(a) The resident's
name;
(b) The resident's ID
number;
(c) The resident's housing
assignment;
(d) The pending
disciplinary infractions;
(e) The
suspension effective date;
(f) The
resident's work site and shift;
(g)
The worksite supervisor's printed name and signature;
(h) Notification to the resident of either:
(1) "You have pled guilty to or been found
guilty of the disciplinary infraction(s) referenced in (4) above. You are
hereby placed in RPS effective (date). All movement and employment restrictions
apply pursuant to Cor 409.05(e) "; or
(2) "You have been found not-guilty of the
disciplinary infraction referenced in (4) above. You will report back to your
work-site effective (date)"; or
(3)
The form shall contain the following language for notification:
"Although you have not been found guilty of a disciplinary infraction, you are being removed from your work assignment permanently per documented unusual circumstances, confidential intelligence information, or first-hand knowledge of individual misbehavior".
(i) The removal in (h)(3) above shall require
the approval of the facilities chief of security or higher authority.
