N.H. Admin. Code § Pos 1103.06 - Surety Indemnification

(a) Until such time the surety indemnification is superseded, the applicant shall submit proof of compliance with RSA 188-D:20-a.

(b) If a surety bond is used to meet this requirement a completed form PEC 1100-02 containing the following information shall be submitted:

(1) Surety bond number;

(2) Applicable license year;

(3) Name of school;

(4) Address of school;

(5) Name of surety company;

(6) Address of surety company; and

(7) Point-of-contact with and telephone number of surety company.

(c) The completed form PEC 1100-02 shall be:

(1) Signed by a school official;

(2) Signed by a surety company agent; and

(3) Dated as to when the form PEC 1100-02 was signed.
