N.H. Admin. Code § Rec 406.04 - Documentation of a Licensee's Participation in Facility-Based Recreation In-Service Training, Including Grand Rounds

Documentation of a licensee's participation in facility-based recreation in-service training, including grand rounds, shall be a written statement on the letterhead of the facility, including:

(a) The name of the licensee;

(b) The title of the in-service training or grand round;

(c) The date and the total number of hours of the in-service training or grand round; and

(d) The signature of the licensee's supervisor at the facility or of the instructor of the in-service training or grand round.
