N.H. Admin. Code § Rec 406.09 - Documentation of a Licensee's Receipt of Special Certifications
(a) Documentation of a licensee's
certification or re-certification in adult, infant and child cardio-pulmonary
resuscitation shall be a photocopy of:
(1)
The front and back of a current cardiopulmonary resuscitation certificate,
known as basic life skills for healthcare providers, issued by the American
Heart Association;
(2) The front
and back of a current certificate in adult cardiopulmonary resuscitation issued
by the American Red Cross together with a current certificate in infant and
child cardiopulmonary resuscitation issued by the American Red Cross;
or
(3) The front and back of a
current certificate in adult, child and infant cardiopulmonary resuscitation
issued by Medic First Aid, by the American Safety and Health Institute or by
any other nationally recognized organization providing training in infant,
child and adult cardiopulmonary resuscitation.
(b) Documentation of any other special
certification shall be a copy of the front and back of the certificate showing:
(1) The licensee's name;
(2) The date of issuance;
(3) The name of the certifying body;
and
(4) The signature of:
a. The instructor of the course leading to
the certification; or
b. A
representative of the certifying body.
