N.H. Admin. Code § Saf-C 311.07 - Forms BDF Schedule of Receipts 2, 2E, 2F, 3, 4

(a) An applicant shall submit the following on forms BDF schedules 2,2E, 2F, 3 and 4;

(1) Distributor's name, license number and federal employer identification number, if applicable;

(2) Schedule numbers as listed below:

a. BDF schedule #2 - Gallons received from New Hampshire licensed distributors in New Hampshire for New Hampshire bulk storage;

b. BDF schedule #2E - Gallons received from New Hampshire licensed distributors in New Hampshire for export;

c. BDF schedule #2F - Gallons received from New Hampshire licensed distributors in New Hampshire and direct shipped to customers;

d. BDF schedule #3 - Gallons imported from another state direct to customer; or

e. BDF schedule #4 - Gallons imported from another state into New Hampshire for bulk storage;

(3) Product type;

(4) Date - month and year;

(5) List of receipts, including:

a. Carrier name and federal employer identification number;

b. Mode of transportation;

c. Points of origin and destination, both city and state;

d. Supplier's name and federal employer federal identification number;

e. Date received;

f. Document number;

g. Gross gallons; and

h. Net gallons; and

(6) Page total.
