N.H. Admin. Code § Saf-C 311.09 - Forms BDF Schedule of Tax-Paid Credits 13C and 13H
(a) An
applicant shall submit the following on forms BDF schedules 13C and 13H:
(1) Distributor's name, license number and
federal employer identification number, if applicable;
(2) Schedule numbers as listed below:
a. BDF schedule #13C - Retail Sales to US
Gov't Armed Forces; or
b. BDF
schedule #13H - Retail Sales to Local or State Gov't and their
agencies;
(3) Product
type;
(4) Date, including month and
year;
(5) List of sales, including:
a. Points of origin and
destination;
b. Purchaser's name
and federal employer identification number;
c. Date delivered;
d. Gross gallons; and
e. Net gallons; and
(6) Page total.
(b) An applicant may submit the following
optional information on forms BDF schedule 13C and 13H:
(1) Carrier name and federal employer
identification number;
(2) Mode of
transportation; or
(3) Document
number.
