N.H. Admin. Code § Saf-C 311.10 - Form AFD 1 Alternative Fuels Dealer Report

(a) An applicant shall submit the following on form AFD 1:

(1) Period covered;

(2) Licensees:

a. Name and address;

b. Telephone number;

c. License number; and

d. Federal employer identification number;

(3) Total sales from schedules 5, 8, and 9;

(4) Tax exempt gallons;

(5) Taxable gallons;

(6) Tax due per product;

(7) Total tax due;

(8) Penalty and interest, if applicable; and

(9) Total amount due.

(b) The preparer shall print his/her name and title, sign, and date the form.
