N.H. Admin. Code § Saf-C 311.12 - Statement of Account
(a) The department's
statement of account shall contain the following:
(1) Company or person's name, address and
federal employer identification number, if applicable;
(2) Account charges or credit, as
applicable;
(3) Account
number;
(4) Date of account charges
or credits, as applicable;
(5)
Account balance;
(6) Reference
number;
(7) Date of statement;
and
(8) Non-financial transaction
number for account credit only.
