N.H. Admin. Code § Saf-C 311.15 - Impending Suspension Notice

(a) The department's impending suspension notice shall contain the following:

(1) Licensee's name, address and federal employer identification number, if applicable;

(2) Account number;

(3) Date of notice;

(4) Statement of action(s);

(5) Statement of available options;

(6) Date by which hearing must be requested;

(7) Date of suspension; and

(8) Dollar amount due on invoice(s)/quarter ending date of unfiled report(s).

(b) The commissioner or his/her designee shall sign the form.
