N.H. Admin. Code § Saf-C 311.21 - Form RT 113, Motor Fuel and Petroleum Products Transporter Report

(a) An applicant shall submit the following on form RT 113:

(1) Licensee's:

a. Name;

b. Physical and mailing address;

c. License number; and

d. Federal employer identification number.

(2) Report period by month and year;

(3) Total gross gallons listed in schedule 14A, as set forth in Saf-C 311.22;

(4) Total gross gallons listed in schedule 14B, as set forth in Saf-C 311.22;

(5) Total gross gallons listed in schedule 14C, as set forth in Saf-C 311.22;

(6) Total of all deliveries in gross gallons; and

(7) Total amount due.

(b) If the licensee completes form RT 112, the licensee shall:

(1) Print his/her title;

(2) Sign and date the form; and

(3) Include his/her telephone number.

(c) If the a person other than the licensee completes form RT 112, the preparer shall:

(1) Sign and date the form; and

(2) Include his/her address and telephone number.
