N.H. Admin. Code § Saf-C 311.22 - Form RT 113 Schedules 14A, 14B and 14C

(a) Form RT 113 schedules 14A, 14B and 14C shall be submitted along with form RT 113, as set forth in Saf-C 311.21.

(b) An applicant shall submit the following on form RT-113 schedules 14A, 14B and 14C:

(1) Company name, license number and federal employer identification number;

(2) Schedule numbers as listed below:

a. RT 113 schedule #14A - Total gross gallons delivered from New Hampshire to out-of-state locations;

b. RT 113 schedule #14B - Total gross gallons delivered from out-of-state locations to New Hampshire; and

c. RT 113 schedule "14C - Total gross gallons delivered from New Hampshire to New Hampshire locations;

(3) Product type;

(4) Date - month and year;

(5) List of deliveries, including:

a. Name and federal employer identification number of the person hiring the carrier;

b. Seller's name and federal employer identification number;

c. Mode of transportation;

d. Points of origin and destination;

e. Name and federal employer identification number of the person taking delivery;

f. Date of delivery;

g. Document number;

h. Gross gallons; and

i. Net gallons; and

(6) Page total.
