N.H. Admin. Code § Saf-C 311.33 - RT 128, Request for Refund on Jet Fuel

(a) An applicant shall submit the following on form RT 128:

(1) Applicant's name and address;

(2) Federal employer identification number, if applicable;

(3) Gallons purchased; and

(4) Gallons paid at $.02 and sold for $.005 or in the case of government agencies, the number of gallons refunded at $.02.

(b) The applicant shall provide a copy of an invoice supporting the number of gallons purchased in (a)(3) above and the invoice shall indicate the amount of road toll charged.

(c) The applicant shall provide a copy of the airline operating certificate, pursuant to Federal Aviation Rule (FAR) Part 121.

(d) The applicant shall print his/her title and sign the form.
