N.H. Admin. Code § Saf-C 311.34 - Form RT 130, Request for Refund on Exempt Credit Cards and Fuel Card Issuers

(a) An applicant shall submit the following on form RT 130:

(1) Applicant's name, address, and telephone number;

(2) Federal employer identification number;

(3) State of New Hampshire Vendor number and remit to, if applicable;

(4) Time period of the claim;

(5) Total gallons from enclosed schedules for gasoline and special fuel;

(6) Dollar amounts of refund request for gasoline and special fuel; and

(7) Grand total of dollar amount of refund request.

(b) The applicant shall print his/her name, title, date, and sign the form.

(c) The preparer, if other than the applicant, shall print his/her name, title, telephone, and email address.
