N.H. Admin. Code § Saf-C 311.37 - Waiver of Statute of Limitations Form

(a) An applicant shall submit the following on the waiver of statute of limitations form:

(1) Certification that the authorized representative can sign the form on behalf of the company or entity;

(2) The entity's name and federal employer identification number, if applicable; and

(3) Preparer's name and title.

(b) The applicant shall sign, date and have the form notarized.
