N.H. Admin. Code § Soil 401.03 - Renewal Application

The applicant shall supply the following information on the "Certified Natural Scientist Renewal Form ":

(a) The applicant's full legal name;

(b) The applicant's business address and telephone number;

(c) The applicant's home address and personal telephone number;

(d) The applicant's e-mail address;

(e) Answer yes or no to the following:

(1) "Have you complied with the continuing education requirements set forth in Soil 403";

(2) "Have you within the past 24 months or not previously reported had any disciplinary or legal action brought against you by any licensing or certification board or jurisdiction";

(3) "Have you within the past 24 months complied with all ethical and professional standards set for the in Soil 500"; and

(4) "Have you within the past 24 months or not previously reported been convicted of any felony, or misdemeanor that has not been annulled by a court pursuant to RSA 651:5 and if not annulled, the name of the court, the details of the offense, the date of conviction, and the sentence imposed"; and

(f) Sign and the date under the following statement:

"I attest that the information contained in this form is true and correct to the best of my knowledge and belief and acknowledge that the provision of false information in the application is a basis for disciplinary action by the Board"
