N.H. Code Admin. R. Saf-C 311.11 - Forms AFD Schedule of Disbursements 5, 8 and 9

(a) An applicant shall submit the following on forms AFD schedules 5, 8, and 9:

(1) Dealer's name, license number and federal employer identification number;

(2) Schedule numbers as listed below:

a. AFD schedule #5 - Gallons delivered - taxable;

b. AFD schedule #8 - Gallons delivered to US Gov't Armed Forces tax exempt; or

c. AFD schedule #9 - Gallons delivered to State/Local Gov't tax exempt;

(3) Product type;

(4) Date, including month and year;

(5) List of sales, including:

a. Mode of transportation;

b. Points of origin, both city and state;

c. Purchaser's name and federal employer identification number;

d. Date sold;

e. Document number;

f. Gross gallons; and

g. Net gallons; and

(6) Page total.
