N.H. Code Admin. R. Saf-C 311.24 - Form RT 115R Schedule of Sales to New Hampshire Retail Dealers

(a) Form RT 115R shall be submitted along with form RT 115, as set forth in Saf-C 311.23.

(b) An applicant shall submit the following on form RT 115R:

(1) Distributor's:

a. Name;

b. Address; and

c. Federal employer identification number, if applicable;

(2) Retail dealer's:

a. Name;

b. Address; and

c. Federal employer identification number, if applicable;

(3) Period covered;

(4) Motor fuel product transporter's name and federal employer identification number;

(5) Mode of transportation;

(6) Origin of product;

(7) From whom product was acquired;

(8) Date product was received;

(9) Document number;

(10) Gross gallons by fuel type;

(11) Grand total of gross gallons; and

(12) Page total.
