N.J. Admin. Code § 13:28-4.5 - Record of practitioners
(a) The holder of a
shop license shall at all times maintain a record of all practitioners
rendering services within the shop that contains the following information for
each practitioner:
1. Full name and any
aliases;
2. Current street address,
including apartment number, if applicable;
3. Telephone number;
4. Social security number; and
5. Date upon which practitioner commenced
services.
(b) The holder
of a shop license and the designated experienced practicing licensee shall have
the documentation maintained pursuant to (a) above immediately available for
inspection on the licensed premises upon request of the Board or its authorized
representative.
(c) The holder of a
shop license shall maintain the documentation pursuant to (a) above for at
least two years from the date upon which each practitioner terminated services.
Such documentation shall be maintained in a safe and secure location on the
premises, in a manner that ensures the confidentiality of a practitioner's
personal information.
